ABSTRACT

Abstract Hyperzincemia and hypercalprotectinemia is a rare inflammatory disease caused by a mutation in the PSTPIP1 gene, with a dysregulation of calprotectin metabolism. Calprotectin is a zinc-binding protein with antimicrobial properties and pro-inflammatory action. The authors report the case of a 20 year-old girl with cutaneous ulcers comparable with pyoderma gangrenosum, growth failure and chronic anemia, who was given the diagnosis of hyperzincemia and hypercalprotectinemia. Measurement of serum zinc and calprotectin concentrations are indicated in these cases.

ABSTRACT

O objetivo foi avaliar a associação entre a concentração sérica de zinco e os fatores cardiometabólicos em crianças pré-púberes brasileiras. Trata-se de um estudo transversal com amostra representativa de escolares na faixa etária de 8 e 9 anos, matriculados em escolas urbanas públicas e privadas em Viçosa, Minas Gerais, Brasil. A composição corporal foi avaliada pela técnica de absortometria de raios-x de dupla energia. Foram avaliadas as concentrações séricas de glicose, insulina, colesterol total, lipoproteínas de alta e de baixa densidade de colesterol, triglicerídeos, apolipoproteínas A (Apo A) e B, ácido úrico, leptina, homocisteína, proteína C reativa-ultrassensível e zinco sérico. A pressão arterial foi aferida por equipamento de insuflação automática. A deficiência de zinco foi observada em 1,3% das crianças. As meninas apresentaram o pior perfil cardiometabólico, com maiores prevalências de valores aumentados para gordura androide, triglicerídeos, resistência à insulina, leptina, zinco e Apo A. No 1º terço de concentração sérica de zinco, a prevalência de resistência à insulina foi 96% maior (RP = 1,96; IC95%: 1,04-3,66) e a de hipercolesterolemia foi 23% menor (RP = 0,77; IC95%: 0,61-0,96) em relação à categoria de referência (2º e 3º terços de concentração sérica de zinco agrupado). Apesar da baixa prevalência da deficiência de zinco, a resistência à insulina foi mais prevalente entre crianças localizadas no menor terço da concentração sérica de zinco. Torna-se importante a prevenção das alterações cardiometabólicas na infância, principalmente da resistência à insulina, com ênfase na avaliação sérica do zinco.


The objective of the study was to assess the association between serum zinc level and cardiometabolic factors in prepubertal Brazilian children. This was a cross-sectional study in a representative sample of schoolchildren 8 to 9 years of age in public and private urban schools in Viçosa, Minas Gerais State, Brazil. Body composition was assessed with dual-energy x-ray absorptiometry. The study measured serum glucose, insulin, total cholesterol, high and low density lipoprotein cholesterol, triglycerides, apolipoproteins A (Apo A) and B, uric acid, leptin, homocysteine, ultrasenstive C-reactive protein, and serum zinc. Arterial pressure was measured with automatic inflation equipment. Zinc deficiency was observed in 1.3% of the children. Girls showed the worst cardiometabolic profile, with higher prevalence of increased android fat, triglycerides, insulin resistance, leptin, zinc, and Apo A. In the first tertile of serum zinc concentration, prevalence of insulin resistance was 96% higher (PR = 1.96; 95%CI: 1.04-3.66) and hypercholesterolemia was 23% lower (PR = 0.77; 95%CI: 0.61-0.96) than in the reference category (grouped 2nd and 3rd tertiles of serum zinc concentration). Despite the low prevalence of zinc deficiency, insulin resistance was more prevalent in children in the lowest third of serum zinc concentration. It is important to prevent cardiometabolic alterations in childhood, especially insulin resistance, with an emphasis on serum zinc level.


El objetivo fue evaluar la asociación entre la concentración sérica de zinc y los factores cardiometabólicos en niñas pre-púberes brasileñas. Se trata de un estudio transversal con una muestra representativa de escolares en la franja de edad de 8 y 9 años, matriculadas en escuelas urbanas públicas y privadas en Viçosa, Minas Gerais, Brasil. La composición corporal se evaluó por la técnica de absorciometría de rayos-x de doble energía. Se evaluaron las concentraciones séricas de glucosa, insulina, colesterol total, lipoproteínas de alta y de baja densidad de colesterol, triglicéridos, apolipoproteínas A (Apo A) y B, ácido úrico, leptina, homocisteína, proteína C reactiva-ultrasensible y zinc sérico. La presión arterial se midió mediante un equipo de insuflación automática. La deficiencia de zinc se observó en un 1,3% de los niños. Las niñas presentaron el peor perfil cardiometabólico, con mayores prevalencias de valores aumentados para la obesidad androide, triglicéridos, resistencia a la insulina, leptina, zinc y Apo A. En el primer tercio de concentración sérica de zinc, la prevalencia de resistencia a la insulina fue un 96% mayor (RP = 1,96; IC95%: 1,04-3,66) y la de hipercolesterolemia fue un 23% menor (RP = 0,77; IC95%: 0,61-0,96), en relación con la categoría de referencia (2º y 3º tercios de concentración sérica de zinc agrupado). A pesar de la baja prevalencia de la deficiencia de zinc, la resistencia a la insulina fue más prevalente entre niñas localizadas en el menor tercio de la concentración sérica de zinc. Es importante la prevención de las alteraciones cardiometabólicas durante la infancia, principalmente de la resistencia a la insulina, con énfasis en la evaluación sérica del zinc.

Resumen El presente estudio buscó identificar en niños preescolares la asociación de las concentraciones de hemoglobina con características socio-ambientales, maternas, biológico-nutricionales y las condiciones de salud; así como con las concentraciones séricas de zinc y retinol. Estudio transversal con muestra de 335 individuos, representativa de los niños de jardines infantiles del municipio de Campina Grande, Paraíba. En los 294 niños estudiados, el modelo ajustado mostró menores concentraciones de hemoglobina en casos de tipo de casa diferente de ladrillo; ausencia de red de escoto; colecta no pública de la basura; no tratamiento del agua para beber; dificultades maternas para leer, escribir o hacer cuentas; edad entre 9-24 meses; peso/edad < - 2 escore Z y problemas de salud en los últimos 15 días, bien como en casos de bajas concentraciones séricas de zinc y retinol. La multicausalidad de concentraciones inferiores de hemoglobina comprendió condiciones socio-ambientales y características materno-infantiles que refuerzan la importancia de medidas que prioricen los niños de menor edad, de madres con menor nivel educacional y con prejuicios en su peso, estado de salud y condición nutricional de otros micronutrientes importantes en el crecimiento.


Abstract This study sought to identify the association between the concentration of hemoglobin and socio-environmental, maternal, biological-nutritional and health condition characteristics in preschool children; as well as the serum concentrations of zinc and retinol. It involved a cross-sectional study with 335 individuals, a representative sample of children enrolled in child day care centers in the city of Campina Grande, Paraíba. In the 294 children studied, the adjusted model showed lower concentrations of hemoglobin when the house was not made of bricks; there was no sewerage system; no public garbage collection; no treatment of drinking water; maternal difficulties for reading, writing and making calculations; age between 9 - 24 months; weight/age < - 2 score Z and health problems in the last 15 days, as well as in cases of lower serum concentrations of zinc and retinol. The multicausality of lower hemoglobin levels included socio-environmental conditions and child-maternal characteristics that reinforce the importance of measures that prioritize the younger children of mothers with lower education levels and prejudiced in weight, health and nutritional status of other important micronutrients for growth.

SUMMARY: The purpose of this study was to compare different methods of maturity evaluation and their relation with performance-related physical and anthropometric variables in young soccer players, with different plasma zinc status. A total of 53 healthy male young soccer players (age: 13±1 years; body mass: 48±10 kg; stature: 160±10 cm) participated in this study. Variables from physical fitness (stature, ST; fat-free mass, FFM; handgrip strength in the dominant hand, DHS), testosterone and zinc plasma concentration were measured. Biological maturity was evaluated by sexual maturity (development of: pubic hair, PH; genitals, GD; axillary hair; AH), bone maturity (TW3 method; BA), and testosterone plasma concentration methods. We observed that: (i) the frequency of hypozincemics and normozincemics children stratified by BA-CA, PH and GD were similar in each category; (ii) the BA allowed the identification of differences between the three categories, in at least two performance-related variables (FFM and DHS, p<0.0001); (iii) the AH method was able to discriminate only for DHS (p<0.0001); and (iv) the testosterone method was not able to identify differences between the four maturation categories with regard to ST, FFM, and DHS. Results suggested that Zinc deficiency did not influence the results obtained for the maturation categories defined by the different assessment methods. The assessment of biological maturation by BA seems to be the most effective for the stratification of performance-related and anthropometric variables in young soccer players. Nevertheless, the AH method should also be considered as a fair option to be used in field studies and practice.


RESUMEN: El objetivo de este estudio fue comparar diferentes métodos de evaluación de madurez y su relación con variables físicas y antropométricas relacionadas con el desempeño en jóvenes jugadores de fútbol, con diferentes niveles plasmáticos de zinc. Participaron en este estudio 53 jugadores de fútbol jóvenes sanos (edad: 13 ± 1 años, masa corporal: 48 ± 10 kg, estatura: 160 ± 10 cm). Se midieron las variables de la aptitud física (estatura, ST, masa libre de grasa, FFM, fuerza de agarre en la mano dominante, DHS), testosterona y concentración plasmática de zinc. Se evaluaron la madurez sexual (desarrollo de vello púbico, PH, genitales, DG, cabello axilar, AH), madurez ósea (método TW3, BA) y concentración plasmática de testosterona. Se observó que: (i) la frecuencia de los niños hipoxincémicos y normozincémicos estratificados por BA-CA, PH y GD fueron similares en cada categoría; (ii) el BA permitió la identificación de diferencias entre las tres categorías, en al menos dos variables relacionadas con el desempeño (FFM y DHS, p <0,0001); (iii) el método AH fue capaz de discriminar sólo para DHS (p <0,0001); y (iv) el método de testosterona no fue capaz de identificar diferencias entre las cuatro categorías de maduración con respecto a ST, FFM y DHS. Los resultados sugirieron que la deficiencia de zinc no influyó en los resultados obtenidos para las categorías de maduración definidas por los diferentes métodos de evaluación. La evaluación de la maduración biológica por BA parece ser la más efectiva para la estratificación de variables de rendimiento y antropométricas en jóvenes jugadores de fútbol. Sin embargo, el método AH también debe ser considerado como una opción justa para ser utilizado en estudios de campo y práctica.

ABSTRACT Objective To investigate the effects of oral zinc supplementation on fatigue intensity and quality of life of patients during chemotherapy for colorectal cancer. Methods A prospective, randomized, double-blinded, placebo-controlled study was conducted with 24 patients on chemotherapy for colorectal adenocarcinoma in a tertiary care public hospital. The study patients received zinc capsules 35mg (Zinc Group, n=10) or placebo (Placebo Group, n=14) orally, twice daily (70mg/day), for 16 weeks, from the immediate postoperative period to the fourth chemotherapy cycle. Approximately 45 days after surgical resection of the tumor, all patients received a chemotherapeutic regimen. Before each of the four cycles of chemotherapy, the Functional Assessment of Chronic Illness Therapy-Fatigue scale was completed. We used a linear mixed model for longitudinal data for statistical analysis. Results The scores of quality of life and fatigue questionnaires were similar between the groups during the chemotherapy cycles. The Placebo Group presented worsening of quality of life and increased fatigue between the first and fourth cycles of chemotherapy, but there were no changes in the scores of quality of life or fatigue in the Zinc Group. Conclusion Zinc supplementation prevented fatigue and maintained quality of life of patients with colorectal cancer on chemotherapy.


RESUMO Objetivo Investigar os efeitos da suplementação oral de zinco sobre a intensidade da fadiga e a qualidade de vida de pacientes durante a quimioterapia para neoplasia colorretal. Métodos Estudo prospectivo, randomizado, controlado e duplo-cego conduzido em um hospital universitário público terciário, com 24 pacientes em regime quimioterápico para adenocarcinoma colorretal. Os pacientes receberam cápsulas de zinco 35mg (Grupo Zinco, n=10) ou placebo (Grupo Placebo, n=14) por via oral, duas vezes ao dia (70mg/dia), durante 16 semanas, desde o período pós-operatório imediato até o quarto ciclo de quimioterapia. Todos os pacientes receberam quimioterapia por aproximadamente 45 dias após a ressecção cirúrgica do tumor. A escala Functional Assessment of Chronic Illness Therapy-Fatigue foi preenchida antes de cada um dos quatro ciclos de quimioterapia. Utilizou-se o modelo de regressão linear misto para dados longitudinais para análise estatística. Resultados Os escores de qualidade de vida e de fadiga foram semelhantes entre os grupos de estudo durante os ciclos de quimioterapia. O Grupo Placebo apresentou piora da qualidade de vida e da fadiga entre o primeiro e o quarto ciclos de quimioterapia, mas não houve mudança nos escores de qualidade de vida e fadiga no Grupo Zinco. Conclusão A suplementação com zinco previne a fadiga e preserva a qualidade de vida de pacientes em quimioterapia para neoplasia colorretal.

ABSTRACT

Toxic metals and deficiency/excess of trace elements can have adverse effects on health. The aim of this study was to quantify toxic metals lead, cadmium and trace elements zinc, copper, aluminium [Al] and Iron [Fe] levels in pregnant women, cord blood and meconium of new-born infants from industrial zones of Karachi, Pakistan. Analytical research was performed from 2011–2012 in low socio-economic pregnant mothers and newborn infants from 20 towns near Sindh Industrial Trading Estates, Federal B industrial area and Korangi industrial areas, Karachi, where environmental pollution was anticipated. Blood samples of pregnant women [n = 416], cord blood [n = 309] and meconium [n = 309] were analyzed quantitatively for metals and trace elements. Results indicated that mothers residing in steel towns were found to have the highest levels of lead. Meconium contained high levels of toxic heavy metals and trace elements compared to cord blood and maternal blood. Maternal blood toxic metals were present in high quantities. Therefore, safety measures should be taken when industrial waste is disposed of in order to prevent population contamination

BACKGROUND: Considering that modification in body composition and growth velocity leads to alterations in zinc and copper nutritional status, the purpose of the study was to assess erythrocyte zinc and serum copper in adolescents of both genders, according to the stage of sexual maturity at different growth phases. METHODS: Two hundred and eighty-six adolescents were assessed, 157 were female, with body mass index Z-scorebetween−2 and +2. Clinical biochemical measurements were assessed using atomic absorption spectrophotometry. Student's t, Kruskal-Wallis, and Bonferroni tests were used. RESULTS Median serum copper was superior, and zinc was inferior to respective reference values in all groups. Serum copper presented significant reduction in the final stages of pubertal development in males; in turn, erythrocyte zinc values increased linearly with the evolution of puberty stages in both genders. Comparison between genders in each stage of growth showed significant difference in peak acceleration and in deceleration for erythrocyte zinc and serum copper, with both being higher in females. CONCLUSIONS: Serum copper is inversely influenced by modifications that occur with the evolution of puberty in males, whereas erythrocyte zinc is directly influenced in both genders, although always with deficient deposition. At peak acceleration and deceleration of growth, there is greater mobilization of erythrocyte zinc and serum copper in males, when compared with females, which does not occur at the onset of the acceleration of growth.

OBJECTIVE: To analyze adherence to the recommended iron, zinc and multivitamin supplementation guidelines for preemies, the factors associated with this adherence, and the influence of adherence on the occurrence of anemia and iron, zinc and vitamin A deficiencies. METHODS: This prospective cohort study followed 58 preemies born in 2014 until they reached six months corrected age. The preemies were followed at a referral secondary health service and represented 63.7% of the preterm infants born that year. Outcomes of interest included high or low adherence to iron, zinc and multivitamin supplementation guidelines; prevalence of anemia; and prevalences of iron, zinc, and vitamin A deficiencies. The prevalence ratios were calculated by Poisson regression. RESULTS: Thirty-eight (65.5%) preemies presented high adherence to micronutrient supplementation guidelines. At six months of corrected age, no preemie had vitamin A deficiency. The prevalences of anemia, iron deficiency and zinc deficiency were higher in the low-adherence group but also concerning in the high-adherence group. Preemies with low adherence to micronutrient supplementation guidelines were 2.5 times more likely to develop anemia and 3.1 times more likely to develop zinc deficiency. Low maternal education level increased the likelihood of nonadherence to all three supplements by 2.2 times. CONCLUSIONS: Low maternal education level was independently associated with low adherence to iron, zinc and vitamin A supplementation guidelines in preemies, which impacted the prevalences of anemia and iron and zinc deficiencies at six months of corrected age.

It is feasible to think that the body composition assessment may be influenced by maturational and zinc status, especially in young athletes, which perform regularly high volume of physical training. In accordance, it seems important to clarify the impact of these factors in body composition assessment in athletes, since errors may lead to mistakes in training prescription and diet elaboration, and therefore affect the athletic performance. The objective was to compare (1) different methods of body composition evaluation in young soccer players stratified by zinc plasma levels; and (2) the two reference methods using skinfolds thickness in children (Slaughter's and Lohman's equations), considering the maturation level. In this cross-sectional study, fifty tree young soccer players (13.3±0.7 y) were submitted to blood collection, electric bioimpedance (BIA), dual energy X-ray absorptiometry (DXA), anthropometric measures (body mass, stature and skinfolds thickness (ST)) and hand-wrist X-ray. Body composition evaluation was performed by: DXA, ST (Lohman and Slaughter equations) and BIA (Houtkooper equation) methods. Zinc status provided two groups: Normozincemic and Hypozincemic athletes, determined by cut-off point of 11.0 µmol/L. Significant difference on descriptive data for all participants after zinc status stratification was observed only for plasma zinc concentration; (2) Significant correlations were observed between the assessment methods (fat percentage: r= 0.34 to 0.98 and p<0.001 to 0.013; fat free mass: r= 0.95 to 0.9998 and p<0.001), and lowers correlations were observed when electric impedance was involved; and (3) Bland-Altman plots across methods showed a closer agreement when DXA and ST were compared. In conclusion (1) The ST method was better than BIA to assess the body composition (in young soccer players) when DXA scans are not available; (2) The comparison of models based on ST showed that the best association with the values from DXA were obtained for the Slaughter equation, followed by the Lohman equation using bone age instead of chronological age; and (3) Plasma zinc levels seem not to influence the body composition assessment, which certainly warrants further studies.


Es factible pensar que la evaluación de la composición corporal puede estar influenciada por el estado de maduración y los niveles plasmáticos de zinc, especialmente en atletas jóvenes, que regularmente realizan un alto volumen de entrenamiento físico. Por tanto, es importante aclarar el impacto de estos factores en la evaluación de la composición corporal de los atletas, ya que errores en su análisis pueden conducir al desarrollo de una equivocada prescripción de entrenamiento, además de una dieta determinada, y por lo tanto afectar el rendimiento deportivo. El objetivo de esta investigación consistió en: (1) comparar los diferentes métodos de evaluación de la composición corporal en futbolistas jóvenes estratificados por los niveles plasmáticos de zinc; (2) comparar los dos métodos de referencia utilizando el espesor de los pliegues cutáneos en niños (ecuaciones de Slaughter y Lohman), teniendo en cuenta el nivel de maduración. En este estudio transversal, cincuenta futbolistas jóvenes (13,3±0,7 años) fueron sometidos a un perfil bioquímico de sangre, bioimpedancia eléctrica (BIA), absorciometría de rayos X de energía dual (DXA), medidas antropométricas (masa corporal, estatura y pliegues cutáneos de espesor (ST)) y radiografía de mano-muñeca. La evaluación de la composición corporal se realizó por: DXA, ST (ecuaciones Lohman y Slaughter) y BIA (ecuación Houtkooper). El nivel de zinc identificó dos grupos: deportistas normozincémicos e hipozincémicos, determinados por un punto de corte de 11,0 mmol/L. Se observó una diferencia significativa en los datos descriptivos de todos los participantes después de la estratificación del estado de zinc sólo para la concentración de zinc en plasma; se observaron correlaciones significativas entre los métodos de evaluación (porcentaje de grasa: r= 0,34 a 0,98 y p <0,001 a 0,013; masa libre de grasa: r= desde 0,95 hasta 0,9998 y p <0,001), y disminuyeron las correlaciones al estar involucrada la impedancia eléctrica. Los resultados a través de los métodos Bland y Altman mostraron un acuerdo más cercano al comparar DXA y ST. El método ST fue mejor que el BIA para evaluar la composición corporal (en los jugadores jóvenes de fútbol), cuando no estaban disponibles los escaneos DXA. La comparación de los modelos basados en ST mostró que la mejor asociación de valores DXA se obtuvieron para la ecuación Slaughter, seguidos por la ecuación Lohman utilizando la edad ósea en lugar de la edad cronológica. Los niveles de zinc en plasma parecen no influir en la evaluación de la composición corporal, lo que amerita más estudios.

ABSTRACT

PURPOSE : This study aimed to determine Cu/Zn ratio, nutritional and inflammatory status in patients during the perioperative period for colorectal cancer. METHODS: The study included patients with histological diagnosis of colorectal adenocarcinoma (Cancer Group, n=46) and healthy volunteers (Control Group, n=28). We determined habitual food intake, body composition, laboratory data of nutritional status, serum calprotectin and plasma Cu and Zn concentrations. Mann-Whitney U-test was performed between-group comparisons and Spearman correlation test for correlations between the variables. RESULTS: Individuals in the Cancer Group presented significantly lower BMI, fat mass, plasma hemoglobin, total protein and albumin as compared with the Control Group. Serum calprotectin[70.1 ng/mL (CI95% 55.8-84.5) vs.53.3 ng/mL (40.3-66.4), p=0.05], plasma Cu concentrations [120 µg/dL(CI95% 114-126) vs. 106 µg/dL(CI95% 98-114), p<0.01] and the Cu/Zn ratio [1.59 (CI95% 1.48-1.71)vs. 1.35 (CI95% 1.23-1.46), p=0.01]were higher in patients with colorectal cancer than in controls. Additionally, the Cancer Group showed negative correlations between the Cu/Zn ratio and Zn intake, hemoglobin, serum albumin, and positive correlation between the Cu/Zn ratio and serum calprotectin. CONCLUSION: These results indicate that an increased plasma Cu/Zn ratio and serum calprotectin, and decreased protein values may be a result of the systemic inflammatory response to the tumor process.

Background Patients with short bowel syndrome have significant fluid and electrolytes loss. Objective Evaluate the mineral and electrolyte status in short bowel syndrome patients receiving intermittent parenteral nutrition or oral feeding. Methods Twenty two adults with short bowel syndrome, of whom 11 were parenteral nutrition dependent (PN group), and the 11 remaining had been weaned off parenteral nutrition for at least 1 year and received all nutrients by oral feeding (OF group). The study also included 14 healthy volunteers paired by age and gender (control group). Food ingestion, anthropometry, serum or plasma levels of sodium, potassium, phosphorus, magnesium, calcium, zinc, iron and copper were evaluated. PN group subjects were evaluated before starting a new parenteral nutrition cycle. Results The levels of sodium, potassium, phosphorus, calcium and zinc were similar between the groups. The magnesium value was lower in the PN group (1.0 ± 0.4 mEq /L) than other groups. Furthermore, this electrolyte was lower in the OF group (1.4 ± 0.3 mEq /L) when compared to the Control group (1.8 ± 0.1 mEq/L). Lower values of copper (69±24 vs 73±26 vs 109±16 µg/dL) were documented, respectively, for the PN and OF groups when compared to the control group. Conclusion Hypomagnesemia and hypocupremia are electrolyte disturbances commonly observed in short bowel syndrome. Patients with massive intestinal resection require monitoring and supplementation in order to prevent magnesium and copper deficiencies. .


Contexto Ressecções intestinais extensas resultam em perda de fluídos e eletrólitos. Objetivo Avaliar os níveis séricos de minerais e eletrólitos em pacientes com síndrome do intestino curto, dependentes ou não de nutrição parenteral. Métodos O estudo incluiu 22 adultos com síndrome de intestino curto, sendo 11 dependentes de nutrição parenteral (Grupo NP) e 11 sujeitos que recebiam todo aporte nutricional por via oral (Grupo VO). Foram incluídos 14 voluntários saudáveis, pareados para a idade e o gênero (Grupo Controle). A avaliação da ingestão alimentar, antropometria, níveis sanguíneos de sódio, potássio, fósforo, magnésio, cálcio, zinco, ferro e cobre foram documentados em todos os voluntários. Resultados Os níveis sanguíneos de sódio, potássio, fósforo, cálcio e zinco foram similares entre os grupos de estudo. Os níveis séricos de magnésio foram menores no Grupo NP (1,0±0,4 mEq/L) em relação aos demais grupos. Além disso, a concentração desse eletrólito foi menor no Grupo VO (1,4±0,3 mEq/L) em relação ao Grupo Controle (1,8±0,1 mEq/L). Foram documentados menores valores cobre (69±24 vs 73±26 vs 109±16 µg/dL) nos grupos NP e VO quando comparados com o Grupo Controle, respectivamente. Conclusão Hipomagnesemia e hipocupremia são distúrbios eletrolíticos comumente observados na síndrome de intestino curto. Os pacientes com ressecção intestinal extensa requerem monitorização e suplementação de magnésio e cobre a fim de prevenir deficiências. .

ABSTRACT

BACKGROUND AND OBJECTIVES: In this study, we aimed to investigate the effects of sevoflurane, desflurane and propofol maintenances on serum levels of selenium, copper, zinc, iron, malondialdehyde, and glutathion peroxidase measurements, and antioxidant capacity. METHODS: 60 patients scheduled for unilateral lower extremity surgery which would be performed with tourniquet under general anesthesia were divided into three groups. Blood samples were collected to determine the baseline serum levels of selenium, copper, zinc, iron, malondialdehyde and glutathion peroxidase. Anesthesia was induced using 2-2.5 mg kg-1 propofol, 1 mg kg-1 lidocaine and 0.6 mg kg-1 rocuronium. In the maintenance of anesthesia, under carrier gas of 50:50% O2:N2O 4 L min-1, 1 MAC sevoflorane was administered to Group S and 1 MAC desflurane to Group D; and under carrier gas of 50:50% O2:air 4 L min-1 6 mg kg h-1 propofol and 1 µg kg h-1 fentanyl infusion were administered to Group P. At postoperative blood specimens were collected again. RESULTS: It was observed that only in Group S and P, levels of MDA decreased at postoperative 48th hour; levels of glutathion peroxidase increased in comparison to the baseline values. Selenium levels decreased in Group S and Group P, zinc levels decreased in Group P, and iron levels decreased in all three groups, and copper levels did not change in any groups in the postoperative period. CONCLUSION: According to the markers of malondialdehyde and glutathion peroxidase, it was concluded that maintenance of general anesthesia using propofol and sevoflurane activated the antioxidant system against oxidative stress and using desflurane had no effects on oxidative stress and antioxidant system. .


JUSTIFICATIVA E OBJETIVOS: Investigar os efeitos da manutenção de sevoflurano, desflurano e propofol sobre nos níveis séricos de selênio, cobre, zinco, ferro e malondialdeído, as mensurações de glutationa peroxidase e a capacidade antioxidante. MÉTODOS: Foram alocados em três grupos 60 pacientes agendados para cirurgia unilateral de membros inferiores, feita com torniquete sob anestesia geral. Amostras de sangue foram coletadas para determinar os níveis séricos basais de selênio, cobre, zinco, ferro, malondialdeído e glutationa peroxidase. A anestesia foi induzida com 2-2,5 mg kg-1 de propofol, 1 mg kg-1 de lidocaína e 0,6 mg kg-1 de rocurônio. Na manutenção da anestesia, sob gás de transporte de 50% O2 e 50% N2O (4 L min-1), sevoflurano a 1 CAM foi administrado ao Grupo S e desflurano a 1 CAM ao Grupo D e, sob gás de transporte em mistura de 50% O2 e 50% ar (4 L min-1), 6 mg kg h-1 de propofol e 1 mg kg h-1 de fentanil foram administrados ao Grupo P. No pós-operatório, amostras de sangue foram novamente coletadas. RESULTADOS: Apenas nos grupos S e P os níveis de MDA diminuíram em 48 horas de pós-operatório; os níveis de glutationa peroxidase aumentaram em comparação com os valores basais. Os níveis de selênio diminuíram no Grupo S e no Grupo P, os níveis de zinco diminuíram no Grupo P, os níveis de ferro diminuíram em todos os grupos e não houve alteração nos níveis de cobre em nenhum grupo no período pós-operatório. CONCLUSÃO: De acordo com os marcadores de malondialdeído e glutationa peroxidase, concluímos que a manutenção da anestesia geral com propofol e sevoflurano ativou o sistema antioxidante contra o estresse oxidativo e o uso de desflurano não teve efeitos sobre o estresse oxidativo e o sistema antioxidante. .

MÉTODOS: Fueron ubicados en 3 grupos 60 pacientes programados para cirugía unilateral de miembros inferiores, realizada con torniquete bajo anestesia general. Fueron recogidas muestras de sangre para determinar los niveles séricos basales de selenio, cobre, cinc, hierro, malondialdehído y glutatión peroxidasa. La anestesia fue inducida con 2-2,5 mg/kg-1 de propofol, 1 mg/kg-1 de lidocaína y 0,6 mg/kg-1 de rocuronio. En el mantenimiento de la anestesia, bajo gas portador de 50% de O2 y 50% de N2O (4 L/min-1), sevoflurano a 1 CAM fue administrado al grupo S; y desflurano a 1 CAM al grupo D y bajo gas portador en mezcla de 50% O2 y 50% aire (4 L/min-1), 6 mg/kg/h-1 de propofol y 1 µg/kg/h-1 de fentanilo fueron administrados al grupo P. En el postoperatorio se recogieron de nuevo muestras de sangre. RESULTADOS: Solamente en los grupos S y P los niveles de malondialdehído disminuyeron en las 48 h del postoperatorio; los niveles de glutatión peroxidasa aumentaron en comparación con los valores basales. Los niveles de selenio disminuyeron en el grupo S y en el grupo P, los niveles de cinc disminuyeron en el grupo P, los de hierro disminuyeron en todos los grupos y no hubo alteración en los niveles de cobre en ningún grupo en el período postoperatorio. CONCLUSIÓN: De acuerdo con los marcadores de malondialdehído y glutatión peroxidasa, llegamos a la conclusión de que el mantenimiento de la anestesia general con propofol y sevoflurano activó el sistema antioxidante contra el estrés oxidativo y el uso de desflurano no tuvo efectos sobre el estrés oxidativo y el sistema antioxidante. .

Introduction We hypothesized that nutritional deficiency would be common in a cohort of postpartum, human immunodeficiency virus (HIV)-infected women and their infants. Methods Weight and height, as well as blood concentrations of retinol, α-tocopherol, ferritin, hemoglobin, and zinc, were measured in mothers after delivery and in their infants at birth and at 6-12 weeks and six months of age. Retinol and α-tocopherol levels were quantified by high performance liquid chromatography, and zinc levels were measured by atomic absorption spectrophotometry. The maternal body mass index during pregnancy was adjusted for gestational age (adjBMI). Results Among the 97 women 19.6% were underweight. Laboratory abnormalities were most frequently observed for the hemoglobin (46.4%), zinc (41.1%), retinol (12.5%) and ferritin (6.5%) levels. Five percent of the women had mean corpuscular hemoglobin concentrations < 31g/dL. The most common deficiency in the infants was α-tocopherol (81%) at birth; however, only 18.5% of infants had deficient levels at six months of age. Large percentages of infants had zinc (36.8%) and retinol (29.5%) deficiencies at birth; however, these percentages decreased to 17.5% and 18.5%, respectively, by six months of age. Objective: This study aims to assess the nutritional status of selenium, copper and zinc; and also the erythrocyte superoxide dismutase activity of HIV-infected children compared to a control group. Methods: A cross-sectional study was carried out with prepubertal HIV-infected children (n = 51) and their healthy siblings (n = 32). All biochemical measurements including plasma selenium, serum copper levels, serum and erythrocyte zinc levels and erythrocyte super-oxide dismutase activity were evaluated according to dietary, clinical and biochemical parameters. Results: Compared to the control group, the HIV-infected children had lower z-score values for height-for-age (p = 0.0006), higher prevalence of stunting (11.8%) (p = 0.047), lower selenium levels (p = 0.0006) and higher copper levels (p = 0.019). No difference was found concerning superoxide dismutase activity (p > 0.05). The HIV-infected group presented a higher proportion (45.1%) of children with zinc intakes below the estimated average requirement (p = 0.014); however, no association with zinc biochemical parameters was found. Conclusion: HIV-infected children have an inadequate selenium and copper nutritional status, which could influence the progression to AIDS. An adequate micronutrient status could improve the clinical conditions in these patients and minimize free radical production and cellular oxidative stress. .

ABSTRACT

El presente estudio buscó analizar la asociación entre la (in)seguridad alimentaria y el estado nutricional de niños preescolares asistidos en jardines infantiles. La seguridad alimentaria fue evaluada a través de la Escala Brasileña de Inseguridad Alimentaria (EBIA). El estado nutricional se evaluó a través del peso/edad, talla/edad, hemoglobina, retinol sérico y zinc sérico. Fueron encontradas prevalencias de déficit de estatura (6,2%), déficit de peso/edad (2,1%), deficiencia de vitamina A (24,4%), anemia (15,5%) y deficiencia de zinc (15,0%). La inseguridad alimentaria familiar fue caracterizada en 64,2% de las familias predominando la forma leve (32,6%). El estudio concluye que la inseguridad alimentaria estimada por la EBIA no se asoció a los Escore-z de crecimiento ni a las concentraciones de retinol sérico, hemoglobina y zinc sérico.


This study sought to examine the association between the food (in)security and nutritional status of preschool children attended in daycare centers. Food security was assessed using the Brazilian Food Insecurity Scale. The nutritional status was evaluated using the weight/height, weight/age, height/age, hemoglobin, serum retinol and serum zinc status. The prevalence of stunting (6.2%), overweight (3.1%), underweight (2.1%), vitamin A deficiency (24.4%), anemia (15.5%), and zinc deficiency (15%) was established. Food insecurity was found in 64.4% of the families, predominantly in its mild form (32.6%). This study concludes that food insecurity as measured by the EBIA was not associated with Z-score growth or with vitamin A, hemoglobin and zinc biochemical concentrations.

ABSTRACT

Zinc (Zn) has been shown to be an essential micronutrient for all organisms including plants, animals and human beings. Deprivation of zinc arrests growth and development and produces multiple system dysfunctions in all these organisms. Because of the involvement of this micronutrient in so many core areas of metabolism, it is common that the features of zinc defi ciency in humans are nonspecifi c with a wide range including growth retardation, alopecia, diarrhea, delayed sexual maturation and impotence, eye and skin lesions and even impaired appetite. Clinical features and laboratory criteria are not always consistent. Th is inconsistency poses a major diffi culty in the search to reliable yet sensitive clinical or functional indicator of zinc status for validation. Further, it has become clear now that the homeostatic mechanisms fall short of perfection and clinically important features of zinc defi ciency can occur with only modest degrees of zinc defi ciency. In this review article we try to look critically at the available tests and various indicators for assessment of zinc’s level for potential requirement and eff ectiveness and try to conclude about the effi cacy of each.

OBJETIVO: Examinar o poder diagnóstico de deficiência de zinco das concentrações capilares desse mineral por meio da comparação com as concentrações séricas. MÉTODOS: Estudo transversal desenvolvido com 170 crianças na faixa etária dos 12 aos 72 meses. A análise da Curva Característica de Operação do Receptor foi utilizada para identificar a sensibilidade e a especificidade do melhor ponto de corte para o diagnóstico da deficiência de zinco no cabelo. Verificou-se ainda a significância estatística da área sob a curva. RESULTADOS:O ponto de corte apontado como ótimo para o zinco no cabelo foi de 292,52ppm (sensibilidade de 55,6% e especificidade de 60,8%), com uma área sob a curva de 0,549, sem significância estatística. CONCLUSÃO: O diagnóstico da deficiência de zinco pelas concentrações capilares desse elemento, em comparação às concentrações séricas de zinco, mostra baixa sensibilidade e ponto de corte ideal superior ao sugerido. Permanece, ainda, a necessidade de novos estudos que possibilitem a obtenção de indicadores bioquímicos mais sensíveis e específicos para o diagnóstico da deficiência de zinco na população, sob tudo sem importantes limitações técnicas...


OBJECTIVE: This study examined the power of hair zinc concentration to diagnose zinc deficiency by comparing hair zinc concentration with serum zinc level. METHODS: This cross-sectional study included 170 children aged 12 to 72 months. The receiver operating characteristic curve was used for determining the sensitivity and specificity of the best cutoff point for diagnosing zinc deficiency in hair. The statistical significance of the area under the curve was also verified. RESULTS: The best cutoff for zinc concentration in hair was 292.52ppm (sensitivity of 55.6%, a specificity of 60.8%), with an area under the curve of 0.549, without statistical significance. CONCLUSION: Diagnosis of zinc deficiency based on hair zinc concentration compared with serum zinc level has low sensitivity and its ideal cutoff is higher than the suggested cutoff. New studies are needed to research more sensitive and specific biochemical indicators of zinc deficiency, especially indicators without important technical limitations...
